Our support to Adult Carers is enhanced through our relationship with many organisations, most notably:
Hillingdon Council provides a large part of our funding for carer support, both through the Combined Carers Services contract and an annual Corporate Grant.
We work closely with colleagues in Adult Social Care, particularly to assess carers needs, either when they first become a carer or when their needs change.
A first assessment for an adult carer providing care for another adult will be done by the Hillingdon Carers Partnership:
However, if you need an assessment of the needs of the person you care for, this can be done as a joint-needs assessment and will be done by Adult Social Care.

The Confederation, Hillingdon CIC provides enhanced primary care services for Hillingdon residents.
They work with 43 Hillingdon GP Practices and local partners to improve patient care through collaborative and integrated service delivery whilst supporting general practice working.
Collaborative Working
The Confederation Hillingdon are working to the good of the people in the local community of Hillingdon and the General Practices that serve that community. They have a positive approach to collaborative working and improving health care in Hillingdon.
Turn2Us helps people in financial need gain access to welfare benefits, charitable grants and other financial help.
Carers Trust Hillingdon is a ‘preferred provider’ for Turn2Us, which means we can process hardship grant payments on their behalf. This saves time as they take our recommendations for grants rather than carers having to be referred to apply for themselves.

Hillingdon Adult Community Learning service
Hillingdon Adult Community Learning (HACL) provides learning opportunities across the Borough for Hillingdon residents.
We are pleased to partner them to offer a variety of workshops and courses just for Carers, whether this be a Hanging Basket workshop or a Computer Skills course.
Carers who enjoy our courses can go on to attend other courses or take a qualification, all of which are highlighted in the in the Adult Community Learning Brochure.
Heathrow Community Trust
Carers Trust Hillingdon has secured funding for a new member of the Young Carers team through Heathrow Community Trust’s Projects for Young People grants programme. The funding will enable us to further develop our School Outreach Programme whilst providing one of our young adult carers with the opportunity to complete her assessed and supported year in employment (ASYE) with us following completion of her social work degree.
We are extremely grateful to be working in partnership with Heathrow Community Trust and this grant will ensure that we can continue to work alongside schools to ensure young carers in Hillingdon are identified, recognised and supported. In addition to one-to-one and small group work with young carers in schools, we undertake staff and governor training, provide volunteer mentors in core subjects, and assist schools to set up their own young carer provision and peer support projects.
Heathrow Community Trust is an independent grant-making charity set up by Heathrow Airport’s owners to support and strengthen local communities close to the airport. In the past three years it has donated more than £1.2 million through its grant programmes, funding projects which support young people, help protect the environment or support active local communities. Funds come from an annual donation from the airport, fines imposed on aircraft that breach noise limits, and Heathrow colleagues who raise funds for us.

Young carers team:
Our Young Carers team has a great range of established partnerships. Some of the main organisations are work closely with are:
We have worked with Create Arts for seven years. A well-loved charity, they aim to empower disadvantaged people through the provision of creative and arts activities.
Our young carers love the school holiday programme they provide and have engaged in a variety of workshops including drama, painting, photography and film- making.
Their most notable successes were their appearance on ITV having ‘reimagined’ the ITV logo and a large mural in The Chimes shopping centre in Uxbridge.
We work closely with P3, a charity that delivers services for socially excluded and vulnerable young people.
They have helped some of our young people with housing issues and they have three homeless hostels in the borough.
They have also delivered some personal development and personal safety workshops for us, such as sexual health and drug awareness and also help with employment and apprenticeship issues.
London Youth works across London to improve the lives of young people. We currently hold their gold, silver, and bronze quality mark awards, which they provide with City and Guilds.
We use their residential centres to enable our young carers to access breaks and have had access to small pots of funding through this fabulous organisation.
The relationship also helps to keep us up to date with any opportunities available for young people in London and any policy changes affecting our young carers.
