
Introduction
Hillingdon Carers Partnership was formed in 2015 to bid for the Combined Carers Services contract, which was awarded to the Partnership in early 2016.
What they offer
The Partnership offers a single point of access for carers. This means that, whatever service or combination of services carers might need, they only have to tell their story once and we will carry out a full Carers Assessment.
Carers can contact any one of the partners and we will communicate with each other to ensure they get what they need. With permission to share their information from the carer, all relevant services will wrap around them forming a circle of support.
With permission, we will also refer carers to other sources of support if they are more appropriate or better meet their needs. Click on each statement to find out more.

A Dementia Support Service is specific to each person's needs and is decided in agreement with them. The service provides one to one support to people with dementia and carers which can be provided face to face, over the telephone or via written communication.
Our Dementia Support service is tailored to each person’s needs and all activity is agreed in advance with the carer, person with dementia and the wider family, if desired.
The service offers one-to-one support to people with dementia and their carers, which can be provided face-to-face, over the telephone or via written communication. The team also offers a home-visiting service for those families that might struggle to get out of the house.
Following an initial assessment, our Dementia Support Workers will provide information, guidance and practical support to help people to better understand dementia, to cope with day-to-day living and to plan for the future. The service also supports people to access other services.
A fuller assessment is completed over time. This enables us to better understand how the person hopes to benefit from our support, to address any money or legal matters, to clarify their personal goals and to plan to ensure they can continue to live well as the dementia progresses.
The team offers a range of opportunities that include training, social activities, Dementia Cafes, singing for the brain sessions, care calling, emotional support and also offers access to a whole range of other support through the Hillingdon Carers Partnership and beyond.

Tovertafel - Magic Tables
Come and visit our Magic Tables and enjoy a series of ‘light’ interactive games developed for people living with dementia and other disabilities.
We welcome both adults and children with special needs to come and discover the magic tables and enjoy them with their families or carers. Organisations are also welcome to make bookings.

We are partners with Carers Trust Hillingdon so there are various services that are dementia specific within the wellbeing service.

Carers Short Breaks
Harlington Care provides well-trained and experienced care workers to take over care for the carer to give them a break. Under the contract, a carer can have up to four hours free replacement (respite) care per week, although this will be based on need as the service is in high demand.
Care workers are trained in all aspects of care and can provide personal care, peg feeding, and can meet many more needs. The initial assessment with the carer and their loved one will ensure that both parties are very clear about what can – and cannot – be provided.
If carers choose, they can top up their respite hours personally, either through a personal budget from Social Services, or their own money if they can afford to.

Forest Therapy is a way of immersing our senses in the atmosphere of the forest for relaxation and positive health and wellness benefits.
Forest therapy groups run on the last Friday of every month between 10am - 1pm. Locations vary.

When caring ends
There is a cafe called The Robins, which takes place on the second Wednesday of the month, in Uxbridge, specifically for carers who have lost the person they care for. We also organise visits to places of interest across the borough.
Give Space was co-founded by drama and movement therapists Rachel and Becci, who are UK Health Care Professions Council (HCPC) registered drama therapists and members of BADth (the British Association of Drama therapists).
Give Space was established in 2020 in response to increasing mental health need. They offer dramatherapy groups one-to-ones and wellbeing workshops.
Dramatherapy is a form of client-led psychotherapy that allows participants to explore their thoughts and feelings, and consider events that may have been difficult. Wellbeing workshops are more skills-based, offering psychoeducation about themes such as managing difficult emotions, body image or exam stress.
